Grant funding will provide case management to eligible Veterans, their Dependents and Surviving Spouses who are low-income. Services: products and equipment that increase, maintain, or improve the beneficiaries' functional capabilities, and promote independent living.
1. distribution of sanitized and refurbished DME donation
2. procurement of new specialized AT items not available in our inventory
3. DME repair services.
Eligible beneficiaries can request services in four ways;

4) Call (210)223-6363, ext. 3 to make an appointment to complete their intake application in person at our
Headquarters & Fulfillment Center or over the phone.
Eligible beneficiaries are not required to make an appointment to complete their intake application, and clients are welcome to visit our Headquarters & Fulfillment Center at 5015 Wurzbach Rd. San Antonio, TX 78238, to walk in and receive services in person.
